Enrollment Agreement and Liability Waiver

ENROLLMENT AGREEMENT AND LIABILITY WAIVER

This agreement is entered into between Santiago Soccer Academy LLC (hereinafter, “the Academy”) and the parent or legal guardian (hereinafter, “the Responsible Party”), for the purpose of formalizing the enrollment of the minor in the sports activities offered by the Academy and establishing the terms of responsibility.

1. FEES AND PAYMENTS

Selected Plan: The Responsible Party agrees to comply with the payment of the training plan chosen at the time of enrollment.

Annual Membership: The Responsible Party accepts the payment of the membership fee established by the Academy, which includes registration and a training uniform.

Payments: All payments must be made on time as scheduled by the Academy. Failure to pay on time may result in late fees and/or suspension of participation.

Refunds: No refunds will be issued for missed practices, games, or special activities.

2. SCHEDULES AND LOCATIONS

The Responsible Party accepts the schedules and training locations selected during the enrollment process.

The Academy reserves the right to modify schedules, training days, or locations due to operational needs, facility maintenance, or circumstances beyond its control. These changes may be made without prior notice, and the Responsible Party acknowledges and accepts this condition.

3. ASSUMPTION OF RISKS

The Responsible Party acknowledges and accepts that participation in sports activities involves inherent risks, including but not limited to accidents and injuries. The Academy and its staff take all reasonable precautions to ensure participant safety; however, such risks cannot be completely eliminated.

4. RELEASE OF LIABILITY

In consideration of the minor’s participation in the Academy, the Responsible Party, on their own behalf and on behalf of the minor, releases and discharges Santiago Soccer Academy LLC, its directors, coaches, employees, volunteers, and affiliates from any liability for accidents, injuries, damages, or losses that may occur during practices, games, events, or any activities related to the Academy.

5. AUTHORIZATION FOR USE OF IMAGE

The Responsible Party authorizes the Academy to capture and use photographs, video recordings, or any other audiovisual material of the minor during practices, games, or Academy events. Such material may be used for promotional purposes on social media, the Academy’s website, or other marketing platforms, without any right to financial compensation.

6. WITHDRAWAL FROM THE ACADEMY

If the Responsible Party wishes to withdraw the minor from the Academy, they must notify the Academy at least 30 days in advance. Otherwise, the monthly fee for the following month will be charged.
